Accessing appropriate services at the right time can be an important step to maintaining independent living. WELLCHECK is a key entry point into Age Concern Stockport as it offers a holistic assessment usually in people's own home, to identify with people what they need to keep independent and in control of their lives. It offers practical information to people about services that could be available to maintain or ideally improve quality of life. WELLCHECK will enable and support individuals to review options, make choices and access services.
It offers practical information about and links to a wide range of topics including:
- Cleaning
- Shopping solutions
- Transport
- Social opportunities
- Laundry
- Living aids
and much more.
WELLCHECK is for people who live in their own homes, council homes or sheltered accommodation. It is available to individuals whether it's just to give the reassurance of knowing what's available; because someone is starting to experience difficulties or following assessment that the person is not eligible for Social Services support. Age Concern Stockport believes in equal opportunities and respects differences of background and circumstances.
With the person's consent referrals can also be made by family, friends and carers, or health and social care professionals working with people of 50 plus.
