Longterm Care

Advice Support & Placement Service

A specialist and independent service on all matters relating to long term care. The Service provides local support, advice and information to people seeking placement into a care home, or people already in care homes who have to move or have concerns.

  • An Independent Service.
  • A Specialist Service for individuals, families, the general public and health care professionals.
  • Professional and sensitive support provided to individuals and their families.
  • Local support based on extensive local knowledge of care homes.
  • Knowledge of local and national legislation pertaining to residential care including funding and charging issues, allowances, assessments, and general issues of concern to older people in the area of long term care.
  • Informal representation and support.
  • An Emergency service supporting placements following the closure or change of registration of homes. 

The service provides:

Advice and information on: 

- choices of residential / nursing care in Stockport and surrounding areas;
- finance and funding issues, for example capital limits;
- specific legal issues such as Powers of Attorney etc.;
- people’s rights in relation to long term care;
- courtesy information/advice packs relating to respite care.

Support for:

- people making decisions about their future care needs;
- people who are considering changing to another home;
- people already resident in a care home who have concerns.

Placement for:

- people moving from hospital into residential or nursing care after an assessment by Social Services.

- people wishing to make arrangements independently of Social Services when privately funding;

The placement is monitored to ensure it is suitable.

The aim of the service is to help older people select a home which is compatible with their needs, wishes and interests.

AND if there is anything we can’t answer or feel there is someone better qualified to help we will refer you on to the right person or agency.
